The Impact of COVID-19

During the coronavirus pandemic, the precautionary efforts made by the Pakistan Society for the Study of Liver Diseases have been significant. All events have been held online through the mode of video conference and new ideas discussed in the same manner. With a webinar dedicated to COVID-19 and liver transplant held on 6th June 2020, the implications of this virus were discussed in relation to the practice of liver transplants. The panel saw some well-qualified individuals, the likes of Professor Faisal Saud Dar, who was also the moderator, Professor Ruben Ciria, Professor Paolo Muiesan, Professor Mohamed Rela, who all discussed different aspects of the topic. Prof. Ruben Ciria, liver transplant and HPB Surgeon, reflected upon the worldwide impact of COVID-19 on liver transplant activity. Prof. Paolo Muiesan, liver transplant and HPB Surgeon, shed light on the donor and recipient issues in the coronavirus pandemic, and Prof Mohamed Rela, also a liver transplant and HPB surgeon, discussed ‘LDLT: emerging out of the current situation’.

A series of webinars were organized to observe World Hepatitis Day. Fifteen million people in Pakistan live with hepatitis B or C. An overwhelming majority of these people are unaware of their disease and lack basic knowledge regarding precaution, prevention, and treatments. Hepatitis has a much higher fatality rate than the Covid-19 pandemic in Pakistan with around 300 people dying every day as a result of hepatitis-related complications. In a webinar organized by PSSLD in association with Express Media Group and moderated by Dr. Zaigham Abbas (President PSSLD), speakers highlighted how there is a need for commitment and concerted action from the government as COVID-19 has further caused some complications in the detection and treatment of hepatitis.

With the coronavirus cases seeing a high after Eid-ul Adha, a sharp drop has followed later. This is good news but the fight with COVID-19 hasn’t ended just yet. According to an article published on Dawn, that highlighted the potential reasons for the drop in numbers, much is unknown to the world regarding the coronavirus. Studies are yet to suggest any hardcore, conclusive evidence, and hence, maintaining all precautionary measures and social distancing are still very much necessary. These values are also seen embodied by the Pakistan Society for the Study of Liver Disease as all in-person activities have been at a halt since the start of the outbreak in the country.

Pakistan Society for the Study of Liver Diseases (PSSLD) organized its 12th Annual Meeting in Karachi 13th to 16th December 2018. The meeting was endorsed by World Gastroenterology Organization (WGO) and American College of Gastroenterology (ACG). The theme of the meeting was “Problem Based Hepatology”.

This was a very well attended and successful meeting that brought together leading researchers and practitioners who delivered informative and thought-provoking lectures, presented research papers and exchanged their knowledge and views so that information on prevention and care of Liver disease could be disseminated to the wider medical and health care community.

Liver diseases are common globally but Pakistan has an enormous share of this problem mainly due to chronic Hepatitis B & C. We are still living in an age where people who otherwise are not suffering from hepatitis are still being exposed to the practice of reuse of disposable equipment, practice of treatment by unsafe injections and ‘drips’ and non-standardized methods used by blood banks.

The organizing committee of 12th Annual Meeting of PSSLD prepared a very innovative and extensive four day scientific program to understand the latest trends of Liver diseases especially new guidelines for the treatment of hepatitis by oral agents, advances in the management of non-alcoholic fatty liver disease (NAFLD), hepatocellular carcinoma, new paradigms and protocols for the treatment of liver diseases.

The international faculty of the conference included the World Gastroenterology Organization (WGO) President, Prof. Cihan Yurdaydin (Turkey), Immediate Past President of The Transplant Society Prof. Nancy Ascher (USA) and the Faculty from the Fellows of American College of Gastroenterology (ACG). The faculty members of the event who came from the USA were Prof. Robert Gish (Stanford), Prof. Nancy Ascher (UCSF), Prof. John P. Robert (UCSF), Prof. Norah Terrault (UCSF), Prof. Mehnaz Shafi (MD Anderson), Dr. Bilal Hameed (UCSF), Dr. Khalid Mumtaz (Ohio), Dr. Faisal Jafri (Kansas), and Dr. Tariq Shakoor. Other international faculty members were Prof. Ting Wu (China), Dr. Lu Wang (China), Prof. Kamil Yalcin Polat (Turkey), Prof. Fazle Akbar (Japan), Dr. Nadeem Tehami (UK), and Dr. Aaron Lim (UK).

Pre-conference hands-on workshop on endoscopy and Interventional procedures related to liver and pancreatic diseases was held at National Institute of Liver and G. I Diseases (NILGID) on 13th December. The second day of the meeting was devoted to the Postgraduate Course on “Living Donor Liver Transplantation (LDLT)” for hepatologists, liver surgeons and postgraduate students. Lectures were delivered by the international and national faculty about the indication, technical aspects, management, and ethical issues related to LDLT.

On the evening of the same day, Joint WGO-PSSLD Symposium was organized with the title of “Viral Hepatitis B & C: Lift the Global Burden”. The symposium was well attended by government officials, non-government organizations, decision-makers, health policy, and planning stakeholders. World Health Organization’s target for elimination of Hepatitis B and C by 2030 appears to be a dream for Pakistan, where about 10 million people are infected with viral hepatitis while a significant number of them die each year due to complications of the infection including end-stage liver disease, liver failure, and liver cancer. Experts from different countries of the world including President of World Gastroenterology Organization (WGO) Prof. Cihan Yurdaydin, Prof. Norah A. Terrault from the United States, Dr. Aaron Lim from the United Kingdom, Dr. Sheikh Muhammad Fazle Akbar from Japan shared different models and strategies to wipe out hepatitis B and C from the world.

President of PSSLD Prof. Masood Siddiq said that Pakistan was among one of the most affected countries due to hepatitis B and C where it was fast spreading among those who inject drugs and by reuse of syringes, transfusion of blood screened by substandard methods, and use of unsterile razors at barber shops. He called for mass screening throughout the country to ascertain the actual data of disease burden and plan treatment of the infected people.

Eminent gastroenterologist, Patron of PSSLD and Director WGO Training Center Karachi Prof. Wasim Jafri said that despite poor situation with regard to Hepatitis B and C in Pakistan, this menace could be eliminated through concentrated efforts and called for joint efforts by the federal, provincial governments and private sector to screen, diagnose and treat viral hepatitis patients in Pakistan.

“Unfortunately, despite the availability of huge resources, our efforts are not concentrated and collaborative and in this way, we are not making any impact. All the stakeholders including federal and provincial governments, private sector and NGOs need to work together to eliminate this menace, which is very much doable”, Prof. Jafri observed.

Chair Organizing Committee and Vice President of PSSLD, Prof. Zaigham Abbas commented on a strategy of micro-elimination. At the moment, there was no hope of Hepatitis B and C elimination from Pakistan by 2030 as there were millions of patients who were still undiagnosed and they were silently spreading these infections to other people, he added. This situation could lead to serious consequences for Pakistan. Community-based micro-elimination strategies to educate, vaccinate, screen, treat, and prevent complications may help in suppressing the spread of hepatitis. “Pakistanis could face travel restrictions in the future where people could be asked to provide evidence that they were hepatitis B and C negative to enter other countries of the world where hepatitis had been eliminated”, he warned and asked for creating mass awareness about these diseases among people so that they could get themselves screened, and treated.

Chair, Hepatology Interest Group, WGO, and Former PSSLD President Prof. Saeed Hamid highlighted his efforts for community-based based screening and treatment to eliminate hepatitis. He deplored that due to a variety of reasons, a large number of people were not opting for treatment despite getting diagnosed for Hepatitis B and C. “In Malir district of Karachi, the prevalence of Hepatitis B and C is around 24 percent which is very alarming. We have worked with community and screened many patients but the majority of them did not turn up for the treatment due to a variety of reasons”, he informed and called for creating awareness among people about hepatitis.

The 3rd day of the event allocated for conference plenary sessions and State-of-the-Art lectures and presentations. There were symposia on hepatitis B, hepatitis C, and hepatocellular carcinoma. The conference dinner was arranged at Mohatta Palace where guest speakers were graced with traditional Sindhi Ajrak and cap gifted by the Society. Prof. Masood Siddiq, President PSSLD, Prof. Zaigham Abbas, Chair Scientific Committee, Dr. Faisal Wasim Ismail, Chair scientific Committee, Prof. Cihan Yurdandin, President WGO and Prof. Nancy Ascher, Immediate Past President, The Transplantation Society made short informal addresses. Prof. Saeed Hamid delivered Oration for Distinguished Achievement Award. Recipient of this Award this year was Prof. Wasim Jafri who thanked all the nomination committee for choosing him. The conference dinner was followed by a musical evening in which several traditional artists performed folk dances. Famous artist and renowned opinion leader Shahzad Roy sang some of his popular songs related to social injustice. He accepted to become an ambassador to raise awareness for hepatitis.

The fourth day of the event started with free paper and poster reviewing sessions followed by symposia on portal hypertension and non-alcoholic and alcoholic fatty liver disease. At the closing ceremony prizes were distributed to the young investigators for the best oral and poster presentation. The services of the sponsors were recognized by awarding them shields. The event closed at lunch.

Taking the problem based aproach, this meeting will help clinicians improve their diagnostic and therapeutic skills in a focused and practical manner. As we continue to grapple with the enormous burden of Hepatitis B and C infections, other conditions are also gaining importance and need our utmost attention. Examples are NAFLD and HCC, and the meeting will therefore also focus on these diseases.

Since its inception 12 years back, PSSLD has managed to hold two very successful APASL Single Topic Conferences in 2009 and 2014, as well as many collaborative meetings with other societies including South Asian Society for the Study of the Liver (2015), Chinese Society of Hepatology (2016), and the EASL International Liver Foundation (2017).

This year, we are expecting a number of academic leaders around the globe. We will be be honored to see you among us in this meeting. We look forward to welcoming you at the 12th PSSLD Annual Meeting.

Climate: In December temperatures in Karachi range from 20C – 25C.

Time Zone: The time zone in Pakistan is Greenwich Mean Time (GMT) + 5 hour.

  • World hepatitis day is observed all over the globe on 28th July every year, aims to raise Global awareness of hepatitis and encourage prevention, diagnosis and treatment.In contribution to this day Pakistan society of Gastroenterology , Pakistan society for the study of Liver disease, Digestive Disease Foundation and Post Graduate Medical Institute/Lahore General Hospital jointly organised a seminar on 28th July 2018 at PC hotel, Lahore.Chief Guest Prof.Dr.Brig.Masood Siddiq gave introductory remarks.Renowned GI personals Prof.Nusrat Ullah ch, Prof.Anwar A khan, Prof Altaf Alam, Prof.Ghias Un Nabi Tayyab, Prof Arif Nadeem and Prof. Shumail Zafar explained the epidemology of Hepatitis in Pakistan and introduce audiance comprising of more than 500 health care professionals including doctors, nurses and paramedics about the new guidelines of Hepatits B and C regarding diagnosis, management plans and related complications. Punjab health department personals elaborated the hepatitis control initiatives by Primary & Secondary health department and Specialised healthcare and medical education department. Hepatitis remains a major cause of morbidity and mortality worldwide. World Health Organization (WHO) recognizes this and has declared 28th July as the World Hepatitis Day. Pakistan Society for Study of Liver Diseases (PSSLD) has been holding seminars and public awareness programs to make the general population aware of the problem for the past 9 years now. A seminar was held in a local Hotel in collaboration with the Pakistan Academy of Family Physicians (PAFP).

Speaking on the occasion, President PSSLD Prof Brig Masood Siddiq introduced to the audience the importance of the day and the significance the WHO places on the planning to eliminate hepatitis from the world. He also talked about the efforts PSSLD had put in to meet the goals set by WHO in Pakistan. He stressed that wide-ranging efforts were urgently needed to prevent millions of new infections and increasing deaths because of it. Specifically he mentioned the wide ranging use of often unjudicious injections and drips, which are amongt eh major reasons for spread of the disease in Pakistan. On the average a Pakistani gets 13 injections per year, which is the highest on the world.

Brig Farrukh Saeed then talked about the incidence of hepatitis in Pakistan and informed the audience that the disease burden in Pakistan was only second to Egypt in the world and that there were about 15 million people suffering from Hepatitis B and C in Pakistan and the prevalence of hepatitis C in the country was around 5 per cent and that of hepatitis B was nearly 2.5 percent of the general population. The usual mode of transmission of hepatitis B and C in Pakistanis is high rate of getting treatment by injections and intravenous therapy procedures, although shaving by unsterilized instruments by barbers and piercings of nose and ears and tattooing also contribute to the problem.

Prof Asghar Aurangzeb Durrani next got on the podium and deliberated on the common features of hepatitis and how it presents; stressing the fact that many an infection may be so mild that it may go unnoticed at all. The most common features of acute hepatitis are nausea, vomiting, pain in abdomen and yellow eye discolouration, with a high coloured urine and loss of appetite.

Dr Junaid Saleem next spoke about the complications of hepatitis. Hepatitis A and E are spread by oral facial route while B, C and delta (D) are spread by blood. The public was informed that the disease may be so mild that it may progress to end stage without the patient ever feeling sick, and then it may suddenly present with life threatening complications for the first time in life. It may cause disease elsewhere too like in kidneys, blood, lungs and brain.

Dr Mohammad Salih talked about the tests needed to diagnose and treat the illness, and when and how should they be ordered by the family physicians. He talked about the relative importance of the tests and their significance.

Prof Muzaffar Latif Gill then ascended the podium and gave an overview of the latest treatment options available in the country, simplifying the treatment options for the family physicians, and laid down an algorithm following which correct drugs can be chosen for the patients. Unfortunately the type of hepatitis present in Pakistan is difficult to treat and can relapse often if not treated with care and diligence.

Prof Brig Amjad Salamat was also present as an expert panelist during the talks. In the end the meeting was summed up by Prof Maj Gen Tassawar Hussian,.stressing upon the doctor community to judiciously use injections and intravenous infusions.

PSSLD Participates in the Annual Meeting of SAASL

Sixth annual meeting of the South Asian Association for the Study of the Liver (SAASL) and fist International Conference of the Nepalese Association for the Study of the Liver (NASL) was held in Kathmandu at Hotel Annapurna, on July 13-14 2018. The theme of the conference was “Alcoholic liver disease”. It was a great academic feast and eminent speakers from South Asian region and abroad attended the meeting. PSSLD actively participated in the meeting. The scientific program focused on latest developments in alcoholic liver diseases, hepatitis B, C, hepatocellular carcinoma etc. State of Art lectures were delivered by eminent international speakers. This scientific meeting broadened the clinical knowledge in liver diseases. Highlights of the meeting were presidential lecture by Dr. Saeed Hamid, the President of SAASL and State of the Art Lecture by Dr. Wasim Jafri. In the inaugural ceremony, Dr. Wasim Jafri was honored with the Life Time Achievement Award of SAASL. This award was conferred by the Honorable Prime Minister of the Nepal, His Excellency.
